APPLICANT REQUESTS THAT:

Her DD Form 214, Certificate of Release or Discharge from Active 
Duty; be amended to reflect the following:

1.  Award of Operations DESERT SHIELD/STORM and Gulf War medals.  

2.  Award of the Air Force Commendation Medal with one Bronze 
Oak Leaf Cluster (AFCM w/1 BOLC).

APPLICANT CONTENDS THAT:

She separated quickly following her return from deployment and 
the awards were not included on her DD Form 214.

The Department of Veterans Affairs (DVA) requires the 
corrections to show she served in Operations DESERT 
SHIELD/STORM. 

In support of her requests, the applicant submits copies of her 
DD Form 214, DD Form 1351-2, Travel Voucher or Subvoucher; AFCM 
citation and narrative and various other documents associated 
with her requests.   

The applicant’s complete submission, with attachments, is at 
Exhibit A.

STATEMENT OF FACTS:

On 1 Sep 81, the applicant entered active duty.

On 30 Apr 91, she was honorably discharged.  She served 9 years, 
7 months and 29 days on active duty.

AIR FORCE EVALUATION:

AFPC/DPSID recommends disapproval for award of the AFCM       
w/1 BOLC.  Although the applicant provided a proposed citation 
and unsigned narrative, DPSID was unable to verify it was placed 
into official channels and was unable to locate a signed 
certificate or special order verifying she was recommended for 
or awarded the AFCM w/1 BOLC.  To grant relief would be contrary 
to the criteria established by DODM 1348.33, Manual of Military 
Decorations and Awards, the Secretary of the Air Force, and the 
Chief of Staff.   

DPSID was able to determine that the applicant should have been 
awarded the Air Force Outstanding Unit Award with four Bronze 
Oak Leaf Clusters (AFOUA w/4 BOLC), National Defense Service 
Medal (NDSM), Southwest Asia Service Medal with two Bronze 
Service Stars (SWASM w/2 BSS), Kuwait Liberation Medal-Saudi 
Arabia (KLM-SA) and the Kuwait Liberation Medal-Kuwait (KLM-K).  
Upon the Board’s final decision, the applicant’s records will be 
administratively corrected to reflect these awards.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

On 25 Apr 14, a copy of the Air Force evaluation was forwarded 
to the applicant for review and comment within 30 days (Exhibit 
D).  As of this date, this office has not received a response.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file.

3.  Insufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice.  We took notice 
of the applicant's complete submission in judging the merits of 
the case; however, we agree with the opinion and recommendation 
of the Air Force office of primary responsibility and adopt the 
rationale expressed as the basis for our conclusion that the 
applicant has not been the victim of an error or injustice.  
Therefore, aside from the administrative corrections for award 
of the AFOUA w/4 BOLC, NDSM, SWASM w/2 BSS, KLM-SA and the KLM-
K), we find no basis to recommend granting the relief sought in 
this application.

THE BOARD DETERMINES THAT:

The applicant be notified that the evidence presented did not 
demonstrate the existence of material error or injustice; that 
that the application will only be reconsidered upon the 
submission of newly discovered relevant evidence not considered 
with this application.
